Once you have an appointment scheduled with a therapist, you will need to fill out the Initial Registration Packet.

Please complete the entire packet and bring it with you to your first visit along with your insurance card if applicable. For teletherapy appointments, completed packets and insurance cards can be submitted via email, mail, or fax (please see below). You can also submit paperwork directly to your individual therapist.
